“Look back” exercise on patients seen by the GP (s) during a certain period may be requested.
A ‘look-back exercise’ is “a retrospective review of the care provided to patients to determine if advice or treatment given was correct and safe, and whether further advice, investigation or treatment is required in response to any shortcomings identified during an investigation.”
PCT advisor may suggest ways of how to review patients seen by the GP(s). He may devise a form and suggest the surgery to invite all patients and do a review on each of them and record on the form. If any patient has moved to another practice, then the surgery must notify both the patient and new GP so that they can look into the note and see the patient and take appropriate action.
